A separate registration must be completed each year, for each sport. Click the blue "Begin Registration" button here to start this process. This will take you to Snap! Mobile to proceed to log in or create an account.

You will add the Medical Eligibility Form page of your completed sports physical exam during this process. This is the last page of the sports physical exam with the title “Medical Eligibility Form” at the top and includes date of exam, Dr. signature and stamp.

Next, click here to upload the Medical Eligibility Form page of your sports physical exam for the Health Office.
